n engl j med 370;17 nejm.org april 24, 2014 1649 requirements that the American Board of Internal Medicine (ABIM) introduced in January 2014 require more effort than previous iterations. Completing NEJM Knowledge+ modules allows clinicians to earn both AMA PRA (American Medical Association Physician’s Recognition Award) Category 1 Credits and up 80 points toward the Self-Evaluation of Medical Knowledge requirement of the ABIM Maintenance of Certification (MOC) program; accumulated MOC points are automatically reported to the ABIM so that clinicians can focus on learning rather than on tracking their activities. Each week, we will post a question from NEJM Knowledge+ to help Journal readers test their understanding and see some of what we’ve created. There’s more information available at the program website, knowledgeplus.NEJM.org. We know that for busy practitioners, time is the most precious commodity. The NEJM Knowledge+ learning system is designed to help clinicians identify what they really know and what they don’t know and then to help fill in the gaps efficiently and effectively. We think this program will benefit physicians as a community of lifelong learners — and will ultimately benefit the patients who rely on us to make the right decision every time.

The effect of selenase on the disease process of patients with septic shock admitted to the intensive care unit: A clinical trial in Tehran

Purpose: Despite discovering new antibiotics, mortality due to septic shock has remained high.   This research has examined the effect of selenase in patients with septic shock admitted to the   intensive care unit (ICU) of a hospital in Tehran.   Materials and Methods: This double-blind clinical trial was done on 80 participants (40 case   and 40 control) who were admitted to the ICU with se...

The effect of physiologic dose of intravenous hydrocortisone in patients with refractory septic shock: a randomized control trial

Objective: Septic shock is a response to infection and tissue hypoperfusion which does not respond to fluid therapy and eventually leads to organ dysfunction. Aggressive treatment of a broad-spectrum antimicrobial and supportive measures are the cornerstones of successful treatment. In addition to the main treatment, there are adjunctive therapies. Steroids are one of the treat...

Effect of Higher Mean Arterial Pressure with Norepinephrine on Tissue Oxygenation and Perfusion in Patients of Septic Shock

The current survival sepsis guideline proposes the use of vasopressors and fluid resuscitation to maintain the mean arterial pressure (MAP) ≥ 65 mmHg. Titrating catecholamine infusion to achieve higher MAP has been demonstrated to improve tissue oxygenation, microcirculation, renal function and overall outcome of the patient in some studies and literature on actual hemodynamic goals is scarce. ...

Optimizing mean arterial pressure in septic shock: a critical reappraisal of the literature

Guidelines recommend that a mean arterial pressure (MAP) value greater than 65 mm Hg should be the initial blood pressure target in septic shock, but what evidence is there to support this statement? We searched Pubmed and Google Scholar by using the key words 'arterial pressure', 'septic shock', and 'norepinephrine' and retrieved human studies published between 1 January 2000 and 31 July 2014....

The Epidemiology, Therapeutic Patterns, Outcome, and Challenges in Managing Septic Shock in a Sub-Saharan African Intensive Care Unit: A Cross-Sectional Study

Background: Septic shock (SS) is a health priority in sub-Saharan Africa. However, there is a dearth of data in this regard. Objective: This study aimed to determine the epidemiology, therapeutic patterns, outcome, and challenges in managing SS in a tertiary intensive care unit (ICU) of sub-Saharan Africa. Methods: The hospital file...

Association of arterial blood pressure and vasopressor load with septic shock mortality: a post hoc analysis of a multicenter trial

INTRODUCTION It is unclear to which level mean arterial blood pressure (MAP) should be increased during septic shock in order to improve outcome. In this study we investigated the association between MAP values of 70 mmHg or higher, vasopressor load, 28-day mortality and disease-related events in septic shock. METHODS This is a post hoc analysis of data of the control group of a multicenter t...
